abstract

background: acute normovolemic hemodilution is an available technique to decrease the side effects of allogenic blood transfusion. the aim of this study was to compare acute normovolemic hemodilution with allogenic blood transfusion in adult patients who were scheduled for femoral fracture surgery. methods: in this randomized controlled trial, 50 patients were randomly allocated into two equal groups. general anesthesia was induced in all patients. after induction of anesthesia in case group (n=25) 500 ml blood was taken from patients, while the circulating blood volume was restored by ringer solution (3ml ringer for each 1ml of blood withdrawn). the blood extracted was re-infused during or after surgery according to the patient's hemoglobin and hemodynamic parameters. in case there was further need for blood, allogenic blood was transfused. in the control group (n=25), only allogenic blood was transfused according to estimated patient’s blood loss. patients' assessment was performed by an anesthesiologist who was blinded to the patients' allocation. results: demographic and laboratory data were similar between both groups.  pre- and postoperative laboratory tests were similar in both groups except in mean platelet count (p=0.001). the patients in the two groups were comparable regarding hemodynamic parameters. in the control group, 11 patients needed allogenic blood transfusion and in the case group, allogenic blood was transfused in 4 patients (p=0.03). conclusion: this study suggested that acute normovolemic hemodilution autotransfusion reduced allogenic blood transfusion in patients who underwent femoral fracture surgery.

Effectiveness of Moderate Acute Normovolemic Hemodilution Combined with Tranexamic Acid on the Reduction of Allogenic Blood Transfusion in Patients Undergoing Off-pump Coronary Artery Bypass

Introduction: There are different approaches to reduce the amount of blood loss and allogenic transfusion in cardiac surgery. Regarding this, the present study aimed to evaluate the blood sparing effect of acute normovolemic hemidilution (ANH) combined with intrao-perative tranexamic acid in patients undergoing off-pump coronary artery bypass (OPCAB).Material and Methods: This study was conduct...

Absence of beneficial effect of acute normovolemic hemodilution combined with aprotinin on allogeneic blood transfusion requirements in cardiac surgery.

BACKGROUND The efficacy of acute normovolemic hemodilution (ANH) in decreasing allogeneic blood requirements remains controversial during cardiac surgery. METHODS In a prospective, randomized study, 80 adult cardiac surgical patients with normal cardiac function and no high risk of ischemic complications were subjected either to ANH, from a mean hematocrit of 43% to 28%, or to a control group...

Does acute normovolemic hemodilution work in cardiac surgery?

To the Editor:—In a recent article by Höhn et al., it was concluded that acute normovolemic hemodilution (ANH) in addition to aprotinin was not beneficial in preventing allogeneic blood transfusions compared to aprotinin alone in cardiac surgery. In this randomized, controlled trial, the patients were hemodiluted to a hematocrit of 28% pre–cardiopulmonary bypass (CPB). The transfusion threshold...

Hemodynamic compensation during acute normovolemic hemodilution.

To the Editor:—Acute normovolemic hemodilution (ANH) causes a reduction in arterial oxygen content because of reduced hemoglobin concentration. The primary compensatory mechanism during ANH is an increase in cardiac output to maintain systemic oxygen delivery. Therefore, the lower limit of an acceptable hemoglobin concentration is related to how low hemoglobin can be reduced without jeopardizin...

Intraoperative minimal acute normovolemic hemodilution in patients undergoing coronary artery bypass surgery.

BACKGROUND & OBJECTIVE Efficacy of minimal acute normovolemic hemodilution (ANH) in avoiding homologous blood transfusion during cardiovascular surgery remains controversial. Postoperative bleeding and transfusion remain a source of morbidity and cost after open heart operations. Our objective was to evaluate the impact of minimal ANH on blood transfusion requirements during open cardiovascular...
